Choose a field

Pick from text, email, phone, date, address, file upload, ratings and scales, choice fields including a choice matrix, layout blocks like headers and dividers, and Captcha so respondents confirm they are human.

Each field type has settings that match what it collects.

Field settings

Set the label, placeholder, help text, and whether the field is required.

Some fields have extra options — file types, date ranges, rating scales, address sub-fields, or matrix rows and columns.

Reorder and remove

On a computer, drag fields to change order. On a phone, reorder from the field list. Remove a field if you no longer need it.
